Report Request Form


Please Note:

All report requests are processed on the Thursday following form submission. Please allow seven (7) to 10 business days for processing and plan requests according to this schedule. Requests submitted during a time of emergency (i.e., significant weather events, natural disasters, civil unrest, etc.) will be handled based on severity and prioritized accordingly through direct contact with the requester.
