Welcome to the Fidelity Feedback page! This page includes a variety of forms used to provide feedback on Gamma Phi Beta’s educational programs. All submissions are reviewed by International Headquarters staff. This means that completing the Fidelity Feedback forms is a simple way you can make your voice heard!
To complete a Fidelity Feedback form, please complete the following steps:
- Locate the educational program you completed/participated in.
- Click on the button to launch the Feedback Form for this program.
- Once in the Feedback Form, use the dropdown to select the specific lessons/training for which you would like to complete the form.
